Or, Acrobat uses whatever email program is specified on your computer and may not be set to the actual email program you use.. Change the default to what you use: 1. Open Control Panel > Internet Properties 2. Click on "Programs" tab 3. Select your email program from "Email" drop-down 4. Click "Ok" Re-open the form and try to email again. It should use the email client you selected.

If it still doesn't work mail the form in.

Payment may be made by MasterCard, Visa, Cash, or Cheque (Cheque made payable to: Steel City Hockey). NSF Cheque Fees are $25.00. Payment along with registration can be dropped off at the Mohawk 4 Ice Center from 9-9pm, Mon to Fri.

50% due with registration (or full payment) balance in post-dated cheque to be paid before the program of your choice starts.

If a Player withdraws for any reason there will NOT be a refund issued. If a player withdraws for a medical reason, a credit will be issued to go towards a future program.

On-Line Registration: Please submit payment within 2 weeks after On-Line registration. Information supplied on this form will be held in the strictest of confidence. 

Parent/Guardian Acknowledgement, I hereby acknowledge that by submitting this Electronic Registration Form, that I certify that the player being registered is physically fit for sports participation and acknowledge full risk related to the program. I give approval to my child(s) participation in all activities of 1315691 Ontario Ltd. O/A Steel City Hockey and assume all risks and hazards incidental to such participation including medical and dental expenses and do waive, release, absolve, indemnify, and agree to hold harmless the company of 1315691 Ontario O/a Steel City Hockey and or it's employees and volunteers. 

I certify that the applicant is in good physical and mental health and is able to participate in a physical vigorous activity. I, the Parent (Guardian) and player also agree to maintain an exemplary code of conduct. Failure to do so will result in the expulsion of the player and parent from 1315691 Ontario O/a Steel City Hockey and no refund or registration fees will be available. 

Participants Must wear full CHA approved hockey equipment during all on ice activities. Participants may move up or down in classes where they will benefit most, after a short evaluation period. Steel City reserves the right to change the dates and times as needed.

All Steel City Hockey programs will run out of the new Mohawk 4 Ice Centre Arena Located at Mohawk Sports Park. The facility offers 4 pads all NHL size ice surfaces & provides traditional year round activities. The facility will have a full service Restaurant & Sports Bar for all post game gatherings. The onsite Sports Store offers Skate Sharpening & a broad selection of products and services. The Arena is over 136,000 sq. feet & has 400 parking spaces.
